Downtown East Toronto Primary Care Provider Resources and e-blasts

The Mid-East Toronto Sub-Region was a local health planning area which supported the development of integrated local systems focused on better health outcomes for local residents. It brought together primary care providers, hospitals, community service organizations, community mental health and addiction agencies and patients/clients to develop a careful understanding of local needs that informed the development and implementation of initiatives that allowed providers to deliver more integrated care for their clients/patients. In other words, it is fostered coordination and integration to improve care outcomes for the population and patient and provider experience. C-HEAL aims to build knowledge across sectors among workers serving individuals experiencing homelessness. Specifically, C-HEAL seeks to build a shared understanding on key issues among frontline workers/supervisors in case management programs, shelters, drop-ins, respites and hospital in Downtown East/Mid-East Toronto.

What is TIP?

Telemedicine IMPACT Plus (TIP) offers one-time interprofessional consultations to patients with complex health needs and their family physician to coordinate care planning and derive new solutions for addressing the patient’s chronic conditions.

The physician, patient, and caregivers also benefit from the support of a dedicated TIP nurse who coordinates the TIP consult and provides support to implement the recommendations resulting from the TIP consult.

Across the Toronto Central LHIN, each TIP consulting team has a core membership (as required) of a:

  • Psychiatrist
  • Internist
  • Pharmacist
  • Social Worker
  • Home and Community Care Coordinator
  • Dietitian

Some of our teams offer specialty consults in:

  • Geriatrics
  • Geriatric Psychiatry
  • Diabetes
  • Endocrinology
  • Developmental Disability

TIP is an OHIP-billable service.

Which patients to refer?

  • Medically complex patients with multiple chronic conditions and medications
  • Frequently hospitalized patients in need of access to psychiatric, mental health or social supports
  • Patients who could benefit from coordinated care planning
  • Patients who have some difficulty attending in-person appointments

Why refer to TIP?

  • Access psychiatric and internist consultation within weeks or sooner
  • Develop a coordinated care plan
  • Navigate health and community resources with a dedicated TIP nurse
  • Gain the necessary supports to help manage the complex patients who “keep you up at night”

Technologies exist that allow for the secure transmission of data to support a seamless patient care journey; however, not all primary care providers have access to these services.

Primary care providers are being offered a select bundle of services through a partnered initiative. By signing up physicians will
