About Us
The Patient and Family Education Program is available across Unity Health Toronto. Our program works with staff, physicians, and learners to create patient education resources, enhance communication, and help patients and families navigate their health care journey.
What we do:
Training and Education
- Tailored education and training on best practices in patient and family education and health literacy
- Education in health literacy, effective communication, teach-back, and plain language
Patient Resources
- Find, create and edit pamphlets, videos, handouts, discharge guides, curriculum and online content
- Resources include an online health library and patient and family learning centres
Collaboration
- Work on various unit, program and corporate project to ensure patient education best practices are embedded
