Accessing Your Health Record

Everyone has a right to access their own personal health information. Personal health information is any information that identifies an individual and connects that individual to receiving care at Unity Health Toronto. It includes information that relates to an individual’s physical or mental health, diagnosis, testing, treatment, family health history, Substitute Decision Makers and care providers. All Health Records Departments within Unity Health Toronto comply with Personal Health Information Protection Act (PHIPA), 2004

There is an administrative fee for copies of your record based on the current fee schedule (set by Health Records on a reasonable cost-recovery basis). An administrative fee of $33.90 ($30 + HST) shall apply to all requestors. This includes pages 1 to 20. After the first 20 pages, any additional pages will cost 25 cents per page + HST. Please ensure that the appropriate processing fee accompanies the request.

Please note: There is no fee to release patient information to other health care facilities or physicians, for continuity of patient care.

The hospital does not issue or copy birth/death certificates. To obtain a birth/death certificate, contact Service Ontario at 1 800 461 2156 or through the Government of Ontario’s Office of the Registrar General.

To release personal health information of a deceased patient, a signed request is required from the executor(s) of the estate (with proof, copy of the signed will). If no will exists, a Certificate of Appointment of Estate Trustee or an Administrator of Estate letter is required. Legal signing authority (POA, SDM) must be submitted with the request for an incapable patient.
