Accessing Your Health Record

Everyone has a right to access their own personal health information. Personal health information is any information that identifies an individual and connects that individual to receiving care at Unity Health Toronto. It includes information that relates to an individual’s physical or mental health, diagnosis, testing, treatment, family health history, Substitute Decision Makers and care providers. 

Please note the following:

  • All Health Records Departments within Unity Health Toronto comply with Personal Health Information Protection Act (PHIPA), 2004. To request access to your personal health information, please complete the Access to Personal Health Information form for the appropriate site and send it to the Health Records Department.
  • Valid photo identification is required to complete your request.
  • There is an administrative fee for copies of your record based on the current fee schedule (legislated by the Toronto Central LHIN and set by Health Records on a reasonable cost-recovery basis). An administrative fee of $33.90 ($30 + HST) shall apply to all requestors. This includes pages 1 to 20. After the first 20 pages, any additional pages will cost 25 cents per page + HST. Please ensure that the appropriate processing fee accompanies the request. There is no fee to release patient information to other health care facilities or physicians, for continuity of patient care.
  • Birth and death records must be requested from the Government of Ontario’s Office of the Registrar General
  • Under PHIPA you have a right (with some exceptions) to request a correction of your own health record. In order to make a correction to your health record, please fill out the Request to Correct Personal Health Information form, clearly outlining what you are requesting to have changed. 

All requests must be submitted with the following Consent to Disclose Personal Health Information and either emailed, mailed, faxed, or hand-delivered to the Health Records Department.
