Partnerships

Unity Health is committed to working collaboratively with our community partners, patients, clients and their families to create the best possible care experiences for everyone involved.

The Ontario government is building a connected health care system centred around patients, families and caregivers.

Ontario Health Teams (OHTs) are a new way of organizing and delivering care that is more connected to patients in their local communities. Health care providers (including hospitals, doctors and home and community care providers) will work as one coordinated team – no matter where they provide care.

This is an exciting time in health care and Unity Health Toronto is very pleased to be a member in several Ontario Health Teams across the city. We will continue to use this space to share updates along with our OHT partners.

West Toronto Ontario Health Team

Our St. Joseph’s Health Centre site is involved in the West Toronto OHT with a number of members. In Year 1, the West Toronto OHT will focus on the following patient populations:

  • People with Mental Health and Addiction needs
  • People with Chronic Disease, with a focus on COPD
  • Frail seniors

Downtown East Toronto Ontario Health Team

Our St. Michael’s Hospital site is involved in the Downtown East Toronto OHT with a number of members. The Downtown East Toronto OHT will focus on the following patient populations:

  • People experiencing homelessness
  • Adults with mental health symptoms who would benefit from counselling or psychotherapy

East Toronto Health Partners (ETHP) Ontario Health Team

Our Providence Healthcare site is one of five anchor partners in the east end of the city. In Year 1, the ETHP will focus on the following patient populations:

  • Seniors and caregivers
  • People living with mental health challenges and addictions
  • Priority Neighbourhoods to meet the local needs of diverse communities

Scarborough Ontario Health Team

Our Providence Healthcare site has joined the Scarborough OHT as one of 31 Formal Alliance Partners together with the Scarborough Family Physician Network. In Year 1, the Scarborough OHT will focus on the following patient populations:

  • Frail seniors living with mental health and/or addiction issues and multiple chronic conditions.

Toronto Health and Social Services Directory

The Toronto Health and Social Services Directory is a website that helps people more easily find health, social and community services in Toronto, including the former municipalities of East York, Etobicoke, North York, Scarborough and York. You can search by location combined with keyword or topic.

The directory has more than 8,000 health, social and community service listings in Toronto. These include services related to mental health, food, housing, employment, youth, seniors, newcomers and more.

The Toronto Health and Social Services Directory is an initiative of the eight Ontario Health Teams serving Toronto. It is meant to complement existing navigation resources in Ontario.
