The Inpatient Consultation Team focuses on the prevention and management of geriatric syndromes such as delirium, dementia, falls, functional decline, frailty, incontinence and medical complexity.

Inpatient Consultation Team Referral Form

Patients 65 years or older who are admitted to Trauma and Neurosurgery or Orthopedics with a fragility fracture will be referred automatically to the Inpatient Consultation Team.
