Accessing Your Health Record

• All Health Records departments within Unity Health Toronto comply with Personal Health Information Protection Act (PHIPA), 2004. To request access to your personal health information, please complete the Access to Personal Health Information form for the appropriate site below and send it to the Health Records department.
• The pre-paid administrative fee
• A copy of valid photo identification is required to complete your request.
• Please note: Patients looking for Diagnostic Images can request their images only by
  contacting Medical Imaging.

There is an administrative fee for copies of your record based on the current fee schedule (set by Health Records on a reasonable cost-recovery basis). An administrative fee of $33.90 ($30 + HST) shall apply to all requestors. This includes pages 1 to 20. After the first 20 pages, any additional pages will cost 25 cents per page + HST. Please ensure that the appropriate processing fee accompanies the request.

Please note: There is no fee to release patient information to other health care facilities or physicians, for continuity of patient care.

The hospital does not issue or copy birth/death certificates. To obtain a birth/death certificate, contact Service Ontario at 1 800 461 2156 or through the Government of Ontario’s Office of the Registrar General.

To release personal health information of a deceased patient, a signed request is required from the executor(s) of the estate (with proof, copy of the signed will). If no will exists, a Certificate of Appointment of Estate Trustee or an Administrator of Estate letter is required. Legal signing authority (POA, SDM) must be submitted with the request for an incapable patient.

For people under 16 years of age, a parent or legal guardian is required to submit the request on your behalf.

Note: Patients looking for diagnostic images can  sign up for online access to their medical imaging records through a secure application called PocketHealth. With this change, St. Michael’s will no longer be providing CDs.
