What type of appointment should I book?
Step 1: Are you experiencing any of the following?
- New-onset chest pain
- Difficulty breathing
- Change in neurological status – new weakness, seizure, slurred speech, head injury.
- Suicidal thoughts or family members concerned about patient safety
- Blood in vomit or stool
- Serious injuries
- Infant under 3 months with fever
- Continuous vomiting and inability to drink fluids
- Pregnant and bleeding or cramping (> 13 weeks)

Please call 911 or go to your nearest Emergency Department
IF NO
Continue to step 2
Step 2: Do you need to book any of the following appointments?
- Childhood vaccines appointments (also know as well baby visits)
- Diabetes care visits (visit with nurse and doctor)
- Blood pressure appointments (visit with nurse and doctor)
- Preventative health/ Physical appointments (this varies by provider)
- Prenatal appointments
