Achieving Seamless Care
We are creating a more integrated system where care is better coordinated, and transitions between our hospital sites and beyond are seamless. We are focusing on strengthening coordination across acute, primary, home and community, post-acute, and specialized care. This will be supported by better information sharing, strong partnerships, and alignment between providers to ensure patients receive the right care at the right time, regardless of where they enter the health system.
Leading Health Equity &
Indigenous Health
We are working to improve the delivery of health care for those who face the greatest disparities, including Indigenous peoples or those who need mental health and addictions services or people experiencing homelessness. This includes enabling the responsible collection and use of demographic information to take targeted actions to improve care, education, and decision-making. We will continue to listen to the needs of our communities to build trust as we undertake this work.
Accelerating Health Innovation
& Digital Excellence
We are leveraging our leadership in research, education, and artificial intelligence to accelerate health system transformation. With game changing national health data platforms such as Vital, we will expand our leadership in AI and big data nationally. We will foster research and innovation and transform health care education through initiatives such as improving patient access to clinical trials and expanding education capacity.
Advancing Specialized Care
We will continue to advance our leadership in delivering highly specialized and complex care, expanding access to programs and services across the network. Efforts will focus on key areas such as minimally invasive cardiac surgery and remote robotic neuroendovascular care through the adoption of new technologies and innovative approaches and on improving timely access to services, including specialized surgical oncology such as hepato-pancreato-biliary (HPB) and thoracic, to ensure patients receive care when they need it most. This work will also strengthen unique programs that support patients with rare and complex conditions.
