Partnering on Ontario Health Teams

    • Building an Ontario Health Team for East Toronto
      Dec. 3, 2019 – This morning, Deputy Premier and Minister of Health Christine Elliott announced that East Toronto Health Partners (ETHP) has been successful in its application to become an Ontario Health Team (OHT) – one of the first 24 OHTs in the province.
      Our Providence Healthcare site is one of the anchor partners in ETHP and we extend our congratulations to everyone across Unity Health involved in the extensive work to date.

The Ontario government is building a connected health care system centred around patients, families and caregivers.

Ontario Health Teams (OHTs) are a new way of organizing and delivering care that is more connected to patients in their local communities. Health care providers (including hospitals, doctors and home and community care providers) will work as one coordinated team – no matter where they provide care.

This is an exciting time in health care and Unity Health Toronto is very pleased to be involved in several Ontario Health Teams opportunities across the city. We will continue to use this space to share updates along with our OHT partners so please check back here often.

West Toronto Ontario Health Team

Our St. Joseph’s Health Centre site is involved in the development of an OHT in the West end of the city with a number of partners who are focusing on identified areas of improvement that will enable a full application in the future.

Year 1 focus patient populations include:

  • People with Mental Health and  Addiction needs
  • People with Chronic Disease, with a focus on COPD
  • Frail seniors

Downtown East Ontario Health Team

Our St. Michael’s Hospital site is involved in the development of an OHT in the downtown core of the city with a number of partners who are focusing on identified areas of improvement that will enable a full application in the future.

Year 1 focus patient populations include:

  • People experiencing homelessness
  • Mental Health (including addictions) with complex health needs

East Toronto Health Partners (ETHP) Ontario Health Team

Our Providence Healthcare site is one of five anchor partners in the east end of the city. The ETHP submitted a full application to the Ministry of Health on Oct. 9. Our submission includes proposed plans and provides detailed evidence of how we will move forward to provide seamless, fully coordinated care for patients.

Year 1 focus patient populations include:

  • Seniors and caregivers
  • People living with mental health challenges and addictions
  • Priority Neighbourhoods to meet the local needs of diverse communities

Scarborough Ontario Health Team

Our Providence Healthcare site has joined the Scarborough OHT as a supporting partner. This OHT was also identified as in development.

Year 1 focus patient populations include:

  • Individuals with Mental Health and  Addiction  needs
  • Individuals with Multiple Chronic Diseases
  • Individuals with Age-Related Frailty
